Sleep disturbances are common among pregnant women and pregnant women with obesity are especially prone to problems sleeping. Disturbed sleep is independently associated with the inflammatory process and adverse pregnancy outcomes. Safe treatment options to enhance sleep during pregnancy are needed. The primary aim of the study was to test Pender's Health Promotion Model (HPM) by evaluating the contribution of physical activity (PA) to sleep parameters in pregnant women, particularly within the context of prepregnant body mass index (BMI). Twenty-nine sedentary women in the second trimester were enrolled in an 8-week PA intervention pilot study and randomly assigned to intervention or control group. Survey data, including the Pregnancy Physical Activity Questionnaire (PPAQ) and the Pittsburgh Sleep Quality Index (PSQI), provided descriptive data. Steps per day, as measured by pedometer, and sleep parameters (sleep onset latency [SOL], wake time after sleep onset [WASO], total sleep time [TST], and sleep quality), obtained from sleep diaries, collected for 3 days at the beginning and end of the program, were used to evaluate the daily interaction between PA levels and sleep in pregnancy. Hierarchical linear modeling (HLM) was used for analysis, as data were nested within persons. No significant change occurred over the course of the study in either PA levels or sleep parameters. Prepregnant BMI contributed negatively to PA levels (p = 0 0033), PA levels were positively predictive (p = 0.037) of sleep onset latency (SOL) and negatively predictive (p = 0.01) of sleep quality, demonstrating a negative effect of PA on sleep when measured daily. These results contradict findings from other studies that evaluated the PA level-sleep relationship over a week or month. Small sample and selection bias may have contributed to these results. The effect of daily PA level on sleep during pregnancy may have time-of-day, dose, and intensity components that need further study.
